Payments are due on the following dates:

December 18, 2009
January 19, 2010
February 19, 2010
March 16, 2010

Club fees include USAV team registration, uniforms, practices, use of practice facilities, tournament entry fees, coach’s expenses, administrative costs, practice equipment, lodging for players and coaches for tournaments which require overnight stays. Forms of Payment
Academy accepts personal checks (made out to: Long Island Volleyball Academy) and all major credit cards. Those paying by credit card should fill out the credit card authorization form and mail to below address.

Terms of Payment
A $25 late fee will be charged for any payment not paid by the due date.  A $30 service fee will be charged for any returned or cancelled check. Players will become immediately ineligible to participate in practice or tournaments if any payment is not received on time, and will remain so until the account is brought current.

Refund Policy & Full Payment Responsibility
The membership fee is non-refundable under all circumstances. If a Member sustains an injury, becomes ill, or for any reason at any time decides not to participate in the Academy program, the Member remains liable for the total season fee. In the event of default, if this obligation is referred to an attorney and/or collection agency, the Member agrees to pay, over and above any liabilities, reasonable attorney fees, court costs, and collection fees.

Long Island Volleyball Academy is committed to providing opportunities for young women in the sport of volleyball. Fundraising programs exist to help subsidize Club Fees and can be organized with your teams coach.
